Nurse's Corner

Dunkerton Parents,

Please refer to your child’s grade level below to find out the requirements for the 2024-25 school year:

PRESCHOOL

  • Proof of Birth Certificate: for verification of age
  • Immunization Record
  • Physical: must be completed by physician.

KINDERGARTEN

  • Proof of Birth Certificate: for verification of age. If it was already given for Preschool enrollment, it is not necessary to be provided again.
  • Immunization Record
  • Physical: must be completed by a physician. Lead level needs to be drawn at this appointment and results are to be given to the school.
  • Certificate of Vision Screening: all students that attended Dunkerton Preschool that had their vision tested through the Lions Club (Kidsight) meet this requirement.
  • Certificate of Dental screening: all students that attended Dunkerton Preschool that had a dental screening through I-Smile meet this requirement.

3rd GRADE

  • Certificate of Vision Screening

7th GRADE

  • Immunizations: all students enrolling in 7th grade need to have an updated Tdap vaccine. This will require a one-time booster dose of tetanus, diphtheria, and acellular pertussis-containing vaccine. All students enrolling in 7th grade need to also have proof of one dose of meningococcal (A, C, W, Y). This must be completed PRIOR to starting the 7th grade. 7th graders will not be able to attend school until this requirement has been met.
  • Concussion form: this is a requirement for ALL students. This form needs to be signed by the parent/guardian and student.
  • Iowa Athletic Pre-Participation Physical Examination: this form needs to be filled out by the student’s primary care doctor if planning to participate in extracurricular activities PRIOR to participation. (Tdap vaccination can be done at time of athletic physical).

8th GRADE

  • Concussion form: this is a requirement for ALL students. This form needs to be signed by the parent/guardian and student.
  • Iowa Athletic Pre-Participation Physical Examination: this form needs to be filled out by the student’s primary care doctor if planning to participate in extracurricular activities PRIOR to participation.

9th GRADE

  • Certificate of Dental Screening: Iowa law requires that any child who is entering 9th grade, or is an out-of-state transfer student, must provide the school with proof of a dental check-up.
  • Concussion form: this is a requirement for ALL students. This form needs to be signed by the parent/guardian and student.
  • Immunization Record: if the student’s immunization record has been updated, this record also needs to be submitted to the school.
  • Physical OR Iowa Athletic Pre-Participation Physical Examination: this is requirement for all incoming 9th graders. This form needs to be filled out by the student’s primary care doctor and turned into the school nurse.

10th GRADE

  • Concussion form: this is a requirement for ALL students. This form needs to be signed by the parent/guardian and student.
  • Iowa Athletic Pre-Participation Physical Examination: this form needs to be filled out by the student’s primary care doctor if planning to participate in extracurricular activities PRIOR to participation.

11th GRADE

  • Concussion form: this is a requirement for ALL students. This form needs to be signed by the parent/guardian and student.
  • Iowa Athletic Pre-Participation Physical Examination: this form needs to be filled out by the student’s primary care doctor if planning to participate in extracurricular activities PRIOR to participation.

12th GRADE

  • Concussion form: this is a requirement for ALL students. This form needs to be signed by the parent/guardian and student.
  • Iowa Athletic Pre-Participation Physical Examination: this form needs to be filled out by the student’s primary care doctor if planning to participate in extracurricular activities PRIOR to participation.
  • Immunizations: all students enrolling in 12th grade and born after September 15, 1999, will need proof of two doses of meningococcal (A, C, W, Y) vaccine; or 1 dose if received when the student was 16 years of age or older. Seniors will not be able to attend school until this requirement has been met.
