Dunkerton Parents,
Please refer to your child’s grade level below to find out the requirements for the 2023-24 school year:
Preschool
- Birth Certificate: Please submit for verification of age.
- Immunization Record
- Physical: Must be completed by a physician. (Form is below or can be supplied by physician’s office.)
Kindergarten
- Birth Certificate: Please submit for verification of age. If it was already given for Dunkerton Preschool enrollment, it is not necessary to be provided again.
- Immunization Record
- Physical: Must be completed by a physician. LEAD LEVEL NEEDS TO BE DRAWN AT THIS APPOINTMENT AND GIVEN TO THE SCHOOL. (Form is below or can be supplied by physician’s office.)
- Certificate of Vision Screening: Iowa law requires that all Kindergartners have a minimum of one vision screening done no earlier than one year prior to enrollment and no later than six months after the date of enrollment. ALL STUDENTS THAT ATTENDED DUNKERTON PRESCHOOL THAT HAD THEIR VISION TESTED THROUGH THE LYONS CLUB DOES MEET THIS REQUIREMENT.
- Certificate of Dental Screening: Iowa law now requires that any child who is entering Kindergarten, or is an out-of-state transfer student, must provide the school with proof of a dental check-up.
3rd Grade
- Certificate of Vision Screening: Iowa law requires that all 3rd graders have a minimum of one vision screening done no earlier than one year prior to enrollment and no later than six months after the date of enrollment.
7th Grade
- Immunizations: All students enrolling in 7th grade need to have an updated Tdap vaccine. This will require a one-time booster dose of tetanus, diphtheria, and acellular pertussis-containing vaccine PRIOR to starting the 7th grade. All students enrolling in 7th grade must also have proof of one dose of meningococcal (A, C, W, Y) vaccine. These vaccines MUST be administered prior to the first day of school.
9TH Grade
- Certificate of Dental Screening: Iowa law now requires that any child who is entering 9th Grade, or is an out-of-state transfer student, must provide the school with proof of a dental check-up.
12th Grade
- Immunizations: All students enrolling in 12th grade and born after September 15, 1999, will need proof of two doses of meningococcal (A, C, W, Y) vaccine; or 1 dose if received when the student was 16 years of age or older.
